Here’s a list of the all-time medal leaders at both the Summer and Winter games from wikipedia. One column includes combined totals of both games.

Although Italy is ranked 3rd all-time, this is actually 4th once all the different Germany’s (East, West, North whatever) are unified. Canada is ranked 16th. Guess who tops the list?
